Form Description
Book of Forms This 7 page document has copies of all of the forms sent home with students on the first day of school including: 1) Welcome letter from principal/vice-principal; 2) Information page about each of the forms in the package; 3) A welcome letter from the school council; 4) A volunteer sign up form; and, 5) A signature sheet that needs to be returned to the school
Lunch Arrangement Form This form that was sent home with students on the first week of school ask parents to indicate if their child or children will be eating lunch at school each day or going home.  This form was to be completed and returned to the school.
School Uniforms This letter from the principal and vice-principal outlines the school's policy on uniforms.
Student Record Card This card was sent home during the first week of school.  It is used to update the school's emergency contact information for all of the children and was to be returned to the school when completed.
